Requests for Applications for Mobile and Hybrid Care, a sub-initiative of Virginia's Rural Health Transformation (RHT) Funding are now open. This sub-initiative focuses on innovative solutions that meet gaps in access to care across rural Virginia.

Funding may support costs related to mobile units, telehealth kiosks, clinics in a box, broader telehealth and service delivery capabilities, home based care models, and additional technology enabled services. Services may include, but are not limited to, primary care, behavioral health and substance use disorder services, dental care, vision care, screenings, diagnostics, preventive care, and specialty care.

For our Planning District, the Request for Applications (RFA) with instructions on how to apply can be found here: https://westpiedmontpdc.org/rural-health-transformation-fund/. Funding for Southside Planning District-based projects (Brunswick, Halifax, and Mecklenburg Counties) are included with Counties in several Planning Districts. WPPD is the lead PDC for this year's projects. There are expected to be three additional years of projects. This year's funding will total $11.8M with a minimum of five projects selected.

To apply, your organization MUST have a Unique Entity Identifier (UEI), obtained from SAM.gov. If you do not have UEI, register as soon as possible for this at SAM.gov. Processing may take several weeks, so you will want to do this as soon as possible.

Deadline for applications is Monday, November 23, 2026 at 5:00 PM.
